HOW TO APPLY Pag-IBIG Loyalty Card Plus ...

Active Pag-IBIG Fund members may easily enrol for a Pag-IBIG Loyalty Card Plus at any of our branches. Here’s how:

1. Pag-IBIG Loyalty Card Plus Application Form. You may download the form by clicking here!
2. Submit the application form at the Pag-IBIG Fund branch nearest you, along with a photocopy of one valid identification (ID) card. Kindly present your ID card for verification. [One (1) photo of valid ID Card for verification.]
3. Card Processing Fee (P125.00)
4. Photo shot and Biometrics.
5. Encoding by bank operator.
